Nobody made a mistake. The approval stalled, and no one had time to chase it.
Those lost days and abandoned treatments quietly erode revenue - missed procedures, unbilled visits, and patients who never return. We built VMOS Prior Authorization for exactly that kind of delay. It is part of Nivaran's Virtual Medical Office Services: professional virtual specialists who log into your EHR and payer portals and do the chasing for you.
Who this is for: Independent practices and multi-location groups that do not want to hire more front-desk or authorization staff yet still need every request submitted complete and followed up on time.
VMOS is outsourced prior authorization management for medications, biologics, procedures, and HMO office visits. Our specialists submit the request, keep tabs on it, follow up, and appeal when the payer allows it. Will it get every request approved? No. It cuts the denials that never should have happened.
Part of Nivaran’s WiderVirtual Operations Suite
Prior authorization is one piece of Nivaran's Virtual Operations offering. Practices also use Nivaran for medical scribing (live, AI-assisted, and hybrid), medical coding, revenue cycle management, insurance verification, referral management, patient registration, appointment confirmation, and call-center support. The same human-in-the-loop model - trained specialists plus technology - underpins the full suite.
What Are Prior Authorization Services?
Prior authorization services are teams that handle payer approvals for a practice. They gather the paperwork, send their request, nudge the payer when it goes quiet, and appeal if the answer is no. The authorization itself (most people just say PA) is the payer’s sign-off that must exist before a medication, procedure, or visit is covered.
Does good prior authorization management protect reimbursement?
It makes sure the approval exists before the visit and matches the claim and that a denial gets a second look rather than a shrug.
Why PriorAuthorization Is Straining Practices
The 2025 AMA Prior Authorization Physician Survey of 1,000 physicians found practices spend around 13 hours a week on PA, with roughly 40 requests per physician weekly.
The rest of the survey is tougher reading. 95% of physicians said PA delays care their patients need. Roughly four in five have watched patients abandon treatment. Burnout? 94% linked it to PA. And nearly three in four said denials have climbed over the past five years.
KFF found that Medicare Advantage insurers made nearly 53 million prior authorization decisions in 2024, fully or partly denying 7.7% of them. Only 11.5% of those denials were appealed, and 80.7% of appeals were overturned at least in part. Plenty of denials that might be reversed sit there, unchallenged.
Under the CMS Interoperability and Prior Authorization Final Rule (CMS-0057-F), specified payers generally have to decide urgent non-drug requests within 72 hours and standard ones within seven days. Drug requests are not covered by those timelines.
What Research on AI-Generated Clinical Text Suggests
In a 2025 study in NPI Digital Medicine, Asgari and colleagues examined 12,999 clinician-annotated sentences from AI-generated clinical notes.
They found hallucinations in 1.47% and omissions in 3.45%; 44% of the hallucinations were judged major. Small numbers- until the text is headed to a payer.
Think of a biologic request where the draft says a previous drug “failed.” The chart says the patient stopped it because of side effects. To a payer, that difference can mean a denial. A person reading the chart catches it.

How VMOSHandles Each Type of Request
Each request type has its own routine in our standard VMOS workflow:
CatchingAuthorization Needs Early
A lot of authorization trouble starts before anyone files a thing. Our eligibility checks run five to seven days before the visit and confirm whether a PA is needed at all. Surgical scheduling checks authorization before booking a procedure. Faxed PA paperwork goes into the right chart, not a stack by the printer.
When the Answer Is No
A denial isn’t the end of the road. Here's what happens:
• If a medication is denied, we tell the provider or designated staff member and suggest covered alternatives where they exist.
• If the payer lets a third party appeal, our specialist starts it. If not, your staff will hear about it right away so the deadline doesn't slip.
• If a procedure needs a peer-to-peer review, we try to set it up. When we can’t, the provider gets the insurer’s contact details.
• Denied office visit requests go to your front desk to follow up.
How to Measure Whether Outsourced Prior Authorization Is Working
Pull these four numbers before the handoff, then again a few months in:
1. Average days from request to payer decision
2. Denial rate by payer
3. Share of denials appealed
4. Share of appeals overturned
